First Eucharist Registration Form
We are excited to announce that registration is now open for children preparing to receive
the Sacraments of First Reconciliation and First Eucharist!
To be eligible for registration, your child should meet the following criteria:
Your family is currently registered at Our Lady of the Assumption Parish.
Your child has completed at least one year of Faith Formation at our parish or a Catholic School and is registered for Faith Formation this year.
Your child has been baptized, and you have their certificate.
Your child will be in Second Grade or above during the 2025-2026 school year.
Considering these points, is your child ready to take this special step in their faith journey and register for First Reconciliation and Eucharist?
