How to Submit Parish Verifications for Catholic Schools

The application process for Catholic Schools has begun and we're beginning to receive Parish Verifications. When submitting your verification, please remember the following:

OLA Catholic School

All verifications must be submitted via our online form at www.olachurch.org/parish-verification. 

Other Elementary Schools

Verifications for other Catholic elementary schools are to be submitted to Karen Kotara, Pastor to the Secretary. You can submit them as follows:

· Dropping off a hard copy of the verification at the Parish Office, or

· Emailing a PDF of the verification to verifications@olachurch.org. (We will not accept photos of the form.)

Catholic High Schools

If your school uses an electronic verification, please have the forms sent to Kare Kotara, Secretary to the Pastor, at verifications@olachurch.org.

Non-Electronic Verifications:

• Drop off a hard copy of the verification at the Parish Office, or

• Email a PDF of the verification to verifications@olachurch.org. (We will not accept photos of the form.)

If you are submitting a parish verification request for a High School - or another Catholic elementary School - and you have children at OLA School - you must submit a separate form to OLA (www.olachurch.org/parish-verification).
