Ministry Reimbursement Forms
All Ministry Spending must have the Approval of the Pastor prior to procurement of items. 

You must contact Meg Warner, Parish Accountant mwarner@olachurch.org to request a Purchase Order.  You will fill out the Purchase Order with estimated expenditures and submit for approval.  Once your Purchase is approved you may submit the Check Request Form and reference the Purchase Order as your authorization.

You have two ways to request Reimbursement for expenses incurred with Ministry work:

1. Check Request Form

2. In-Kind Donation Form (used for Tax purposes)

Please allow 10 days turnaround for Forms.  Return to the parish office:  Attention Meg Warner
