Note: Some of the forms will seem repetitive and/or unrelated to your volunteer position. All of these forms are required by the Archdiocese of Atlanta.

REIMBURSEMENT FORMS:

The following forms are for use with ministry activities that you may be involved in.  After completing, please attach all original receipts and forward to the attention of Meg Warner, Parish Accounting. if you have any questions please contact Meg at ext 130 or mwarner@olachurch.org  Please note that all items for reimbursement must not be comingled with personal items.  The receipt must contain only parish related items:
